Fever after Kawasaki disease can recur; this is an important part of post-discharge care. Even after treatment and defervescence, some children may experience a return of fever if inflammation flares again or if there is IVIG resistance. This possibility means caregivers should monitor temperature and seek medical evaluation if fever returns, rather than assuming the illness is over or ignoring it.

Antibiotics aren’t automatically appropriate for this fever unless there’s a separate infection identified.
